Nominate a representative

On 1 December 2015 the law in Wales changed to bring in a soft ’opt-out’ system for consent to organ donation.

If you don't want to make a decision about donating your organ, you can appoint someone to do that for you.

If you die, we will ask this person(s) if your organs should be donated.

How to register your decision

You are able to register your decision and record the name(s) of your appointed representative(s) on the Organ Donor Register.

Or you can complete an Appointing a Representative form.

After you die

  • If we cannot contact your representative, we will ask your family to make the decision about organ donation
  • If we cannot contact your family, your organs will not be donated
